How to get started with our services
You will need to take a couple of steps in order to get ready for services with Firefly. First, you will need to review our program descriptions to start identifying which program will be best for your child and your family. If you are unsure, someone from our office can further discuss our programs with you once you have sent in the Intake Packet.
Next, you will need to complete the Intake Packet which you can download from this site. You will find a link to the PDF file on this page under “Forms and Information.” If your child is already receiving services, but you would like to begin a new program, please complete the Therapy Request Form.
Once you complete your Intake Packet, you can submit this to Firefly by mail, email, or fax. Please email Intake Packets to our Clinical Liaison, at email@example.com. Our address and fax number are at the top of the Intake Packet.
After we receive your Intake Packet, we will contact you for additional information, to verify your program of interest, and to discuss insurance and payment options. The start date of your services will depend on our waitlist, which will vary greatly between each program.
Initial Consult Process
Before we begin any services, you will need to work together with our Clinical Liaison to verify how you would like to pay for services. This step is crucial before beginning any services because most insurance providers will require us to obtain authorization before submitting any claims. If we skip this step, it will be difficult to backtrack.
When we are ready to begin services, we will take you through our Initial Consult Process. The Initial Consult will help us to gain a better picture of your child, which will allow the consultant working with you to verify that the program is a good fit for your child. In addition, most insurance companies require that we submit a report after an initial number of hours have been spent with your child which includes our recommendations for moving forward.
The Initial Consult takes place in two parts. First, you will come to Firefly for a parent interview with one of our consultants. At this time, your consultant will schedule a second meeting in order to meet and observe your child. During the Initial Consult, you will also have a chance to meet with our Clinical Liaison in order to discuss financial questions and processes.

My son, Declan, was diagnosed with autism on June 22, 2006. He was 26 months at the time. While not surprised, I was shocked that this was now a reality. Declan had always been a very sensitive baby from the time he was born. At six hours old, my husband and I knew something was different about him. The nurses at St. Joseph's hospital kept kicking him out of the nursery for screaming so loudly he was upsetting the other newborns. We started calling him the boomerang baby.Read Full Story
